Position Summary: As a Nephrology Nurse Practitioner, you will play a vital role in delivering comprehensive care to patients with kidney-related conditions. You will work closely with our nephrologists and healthcare team to manage patient care, conduct assessments, and implement treatment plans.

Key Responsibilities:

• Conduct thorough patient assessments and physical examinations.
• Manage and provide care for patients undergoing dialysis.
• Perform dialysis rounds in outpatient setting.
• Diagnose and manage acute and chronic kidney diseases.
• Develop and implement individualized care plans.
• Order and interpret diagnostic tests and labs.
• Prescribe medications and treatments as appropriate.
• Educate patients and their families about kidney health and disease management.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Master’s degree in Nursing from an accredited program.
• Current Nurse Practitioner license in PA
• Certification as a Nurse Practitioner with a focus in nephrology preferred.
• Minimum of 5 years of experience in nephrology or related field.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
