POSITION PURPOSE
The Community Healthcare Worker (CHW) plays a vital role in improving health outcomes by providing in-home support, care coordination, health education, and community resource navigation. This role blends direct community engagement with patient advocacy and interdisciplinary collaboration to reduce barriers to care and promote wellness.
The CHW supports physical, behavioral, and social health needs by conducting home visits, assisting with care plan adherence, and connecting members to critical community services. The ideal candidate demonstrates compassion, cultural humility, and a commitment to serving underserved populations. This role requires regular travel throughout assigned counties and surrounding service areas.
PRN EMPLOYMENT STATUS
This is a Per Diem (PRN) position and is scheduled on an “as-needed” basis to support operational staffing needs. PRN employees may be utilized to cover planned and unplanned absences including call-offs, vacations, extended leave, and vacant positions. Adobe Population Health is not obligated to offer shifts, and PRN employees are not guaranteed consistent scheduling beyond program participation requirements.
DUTIES & RESPONSIBILITIES
Care Coordination & Patient Support
- Conduct field-based home visits to assess social, environmental, and basic health needs.
- Assist patients in navigating healthcare systems and community services.
- Support scheduling of appointments, care follow-ups, transportation, and referrals.
- Provide patient check-ins via phone, virtual platforms, and in-person visits.
- Advocate for patient access to services, acting as a liaison with providers and support agencies.
- Assist with addressing care gaps and supporting preventative screenings.
Health Education & Empowerment
- Provide education on chronic disease prevention, wellness, nutrition, medication safety, and lifestyle management.
- Encourage patient engagement and empowerment in their care plans.
- Offer emotional support and guidance to help patients manage life transitions and stressors.
Community Outreach & Resource Navigation
- Connect patients to community programs including housing, food security, behavioral health, financial assistance, and other social supports.
- Participate in community events, health fairs, and outreach programs.
- Conduct home safety evaluations and recommend resources to support independent living.
- Build relationships with community partners and collaborate to expand support networks.
Documentation & Reporting
- Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
- Track patient interactions, referrals, and service outcomes.
- Ensure compliance with HIPAA and organizational confidentiality requirements.
- Participate in quality improvement and program evaluation activities.
Operational & Team Support
- Collaborate with interdisciplinary teams, including clinical and social service staff.
- Attend team meetings, trainings, and case review sessions.
- Support setup and breakdown for community health events.
- Perform additional related duties as assigned.
PRN Coverage Expectations
- Maintain availability for a minimum of one (1) eight-hour shift per week.
- Work a minimum of four (4) shifts per month to remain active in the PRN Staffing Program.
- Respond and assist with staffing requests at least three (3) out of every four (4) times contacted when reasonably available.
- Report to assigned shifts at scheduled start times or within two (2) hours when covering unplanned absences.
- Accept assignments within designated service areas as directed by leadership.
- Notify management in advance of any known periods of unavailability following company policy.

EDUCATION, LICENSES, & CERTIFICATIONS
- High school diploma or GED required.
- An associate or bachelor degree in a related field preferred.
- Medical Assistant (MA), CNA, EMT, or other healthcare background a plus.
- Current CPR/BLS certification preferred.
- Valid driver’s license and proof of insurance required.
COMPENSATION & BENEFITS (PRN)
PRN employees are compensated at a premium hourly rate and are not eligible for benefits including but not limited to paid time off, holiday pay, bereavement leave, medical, dental, vision, life insurance, or disability insurance. Workers’ Compensation coverage is provided in accordance with state and federal regulations. PRN employees are not eligible for annual salary adjustments.

- Must be able to frequently drive long distances, which may require a two-hour radius of your designated county (travel time is paid and mileage reimbursement is available).
