Overview of the Role
The Nurse is accountable for employing the nursing process to deliver care to Avera@Home patients, adhering to the ANA Standards of Clinical Nursing Practice. This role involves taking responsibility for the outcomes of nursing care provided in patients' homes or healthcare facilities. The Nurse must possess the necessary knowledge and skills to deliver outstanding care to the designated population.
Key Responsibilities
- Gather patient health information.
- Evaluate assessment data to identify nursing diagnoses and focus areas.
- Establish individualized expected outcomes and goals for each patient.
- Create a care plan that outlines interventions to achieve the desired outcomes.
- Execute the interventions specified in the care plan.
- Assess the patient's progress towards achieving the outcomes.
- Continuously evaluate the quality and effectiveness of nursing practices.
- Collaborate with patients, their families, and healthcare professionals to provide comprehensive care.
- Incorporate research and evidence-based practices into care when applicable.
- Consider safety, effectiveness, and cost factors in planning and delivering patient care.

Required Education, Licenses/Certifications, or Experience:
- Valid Driver's License - This position necessitates a current driver's license and the ability to be insured by the organization's automobile liability insurance upon hiring.
- Registered Nurse (RN) - An active nursing license in the state of practice is required upon hiring.
Preferred Education, Licenses/Certifications, or Experience:
- A minimum of two (2) years of relevant experience and/or training, or an equivalent combination of education and experience.
