Position: Registered Nurse (RN)
Location: Westminster, CO (and surrounding territory)
Employment Type: Full-Time ( Per Visit)
Compensation: Salary + Sign on + benefits
Position Summary
The RN provides skilled nursing care to help patients achieve their highest level of independence and well-being in their homes. The RN also prepares patients and caregivers for a safe continuum of care when independent disease management is not possible.
Key Responsibilities
- Conduct initial and ongoing patient assessments, including OASIS documentation.
- Develop and update individualized care plans.
- Administer medications, wound care, phlebotomy, and other skilled nursing services.
- Educate patients and caregivers on disease management and home care.
- Collaborate with physicians and interdisciplinary care teams.
- Supervise LPN/LVNs and Home Health Aides as required.
- Complete accurate and timely electronic documentation.
- Maintain compliance with all regulatory and company standards.
Qualifications
Required
- Graduate of an accredited Diploma, Associate, or BSN Nursing Program.
- Active Colorado RN license.
- Current BLS/CPR certification.
- Minimum one (1) year of RN clinical experience.
- Valid driver's license, reliable transportation, and current auto insurance.
Preferred
- Home Health nursing experience.
- OASIS documentation experience.
- ER or ICU experience.
