Job DescriptionJob Opportunity: Licensed Practical Nurse (LPN)

Our organization is in search of a caring and committed Licensed Practical Nurse (LPN) to become a vital member of our Healthcare & Medical Services team. In this role, you will be instrumental in providing exceptional, resident-focused care within a collaborative, interdisciplinary setting. Your responsibilities will include delivering direct bedside nursing care to residents and patients, addressing their physical, emotional, and psychosocial needs while adhering to our mission of transforming healthcare through innovation, continuous improvement, and cultural enrichment.

Key Responsibilities

  • Conduct ongoing assessments of patients and document vital signs.
  • Administer medications and treatments as prescribed.
  • Assist with wound care, catheter care, and other clinical procedures within your state-approved scope of practice.
  • Contribute to the creation and execution of personalized care plans.
  • Monitor residents for any changes in their condition and promptly communicate findings to the supervising RN or provider.
  • Participate in care conferences to ensure that each resident's needs and preferences are met.
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR).
  • Adhere strictly to infection control practices, safety protocols, and HIPAA regulations.

Collaboration and Communication

As an LPN, you will work closely with RNs, CNAs, therapists, physicians, and other members of the care team to coordinate services, encourage early intervention, and support favorable clinical outcomes. You will also provide clear communication and education to residents and their families regarding medications, treatments, disease processes, and self-care strategies, empowering them to make informed decisions about their care. Additionally, you will assist with admissions, transfers, and discharges, ensuring a smooth and respectful transition for each resident.
