Job Type: Full-Time (Onsite / Field-Based)
Schedule: Monday - Friday
Practice Setting: Advanced Wound Care (Home Health & Outpatient)

Job Overview:

We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join a growing healthcare team specializing in advanced wound care. This role focuses on delivering high-quality, patient-centered care through a combination of home visits and outpatient services.

This is an excellent opportunity for both new graduates and experienced providers who are passionate about procedural care, improving patient outcomes, and working in a collaborative clinical environment.

Key Responsibilities:

• Perform comprehensive patient assessments, including physical exams and detailed wound evaluations

• Diagnose and manage acute and chronic wounds (e.g., diabetic ulcers, pressure ulcers, post-surgical wounds)

• Provide advanced wound care treatments including debridement, dressing management, and infection control

• Perform clinical procedures such as suturing, IV therapy, phlebotomy, and catheterization

• Collaborate with physicians and interdisciplinary teams to develop individualized care plans

• Conduct home visits and ensure continuity of care through follow-ups

• Document patient encounters accurately using EHR/EMR systems (e.g., Epic, Cerner, Athenahealth)

• Educate patients and caregivers on wound care, infection prevention, and lifestyle modifications

• Participate in quality improvement initiatives and clinical programs

Qualifications:

• Active and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license

• Certification as NP or PA required

• New graduates are welcome to apply

• Strong understanding of anatomy, physiology, and wound healing principles

• Knowledge of infection control practices

• Experience with clinical procedures (suturing, IV therapy, catheterization) preferred

• Proficiency with EMR/EHR systems

• Familiarity with CPT coding and workers' compensation (preferred)

• Strong communication and patient education skills

• Ability to work independently in a field-based/home health setting
