WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …


We are seeking an experienced Case Manager to join our High Acuity Children’s Services team. The Case Manager is responsible for actively engaging and collaborating with clients and their families/guardians to reduce barriers to achieving positive outcomes in critical areas of life. This crucial role will provide community, home, and office-based clinical case management services to children with psychiatric illnesses.


Main Duties and Responsibilities:

  • Provide community, home, and office-based case management services to children and their families.
  • Participate in daily team meetings to review client visits, concerns, and coordinate care.
  • Co-facilitate family sessions and collaborate on clinical recovery goals with families.
  • Work toward stabilizing and maintaining children in their homes and communities.
  • Develop strength-based comprehensive assessments and recovery plans.
  • Apply evidence-based practice concepts to aid clients in understanding and managing their symptoms.
  • Maintain family and collateral contacts to ensure quality, client-centered services.
  • Assess and involve natural supports in recovery planning and discharge processes.
  • Provide primary case management, and crisis intervention support as indicated, collaborating closely with assigned clinicians during crises.
  • Demonstrate competency in child development, substance use, and provide trauma-sensitive and culturally competent care.
  • Utilize Electronic Health Records (EHR) to document all services, client and collateral contacts, and appointments.
  • Attend all agency-required meetings and agency trainings.
  • Participate in individual and/or group supervision for therapeutic and problem-solving techniques.

Requirements:

WHAT WE EXPECT OF YOU…

  • Minimum of 1-2 years of experience in the Human Services field, children’s experience preferred.
  • Associate Degree in a related Human Services field required, Bachelor Degree preferred.
  • Exceptional verbal and writing skills with the ability to communicate with audiences across all levels.
  • Self-motivated and independent, with the ability to prioritize work to meet deadlines.
  • Valid Driver’s License with reliable transportation to travel to our facilities throughout the area.
