The Role: Healthcare Navigator, SSVF
What You'll Do: You'll work with Veterans with complex needs who require assistance accessing health care services and adhering to health care plans. The main goal of this position is to improve access to needed health care. You'll provide case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation to Veterans. You'll connect Veterans to health care benefits, working closely with their primary care provider and members of their interdisciplinary treatment team. You'll act as a liaison between our SSVF Program, the VA and/or community medical clinics.
- Conduct NON-CLINICAL assessments to assess strengths, limitations, risk factors, and internal/external supports and service needs to optimize the Veteran's ability to access and maintain health care services
- Maintain electronic records and/or equivalent system to include the Veteran's record and Homeless Management Information System (HMIS)
- Advocate and work closely with Veterans in communicating preferences in care and personal health-related goals
- Serves as a resource for education and support for Veterans and families and help identify appropriate and credible resources and support tailored to the needs and desires of the Veteran
- Assist Veterans in gaining access to health care, gaining entry to VA and/or community health care (including mental health/substance use treatment)
- Develops and implements supporting healthcare plans that identify barriers to treatment; problem-solve barriers to care (i.e. transportation, childcare)
- Provides education on wellness-related topics by providing pamphlets or other literature on smoking cessation, diabetes management and exercise; inviting guest speakers to education groups on health-related issues for Veterans, and linking Veterans to support groups or other programs in the VA or community to support health goals.
- Connects Veterans to VA health care by working with the VAMC to facilitate enrollment, gathering documentation and completing required paperwork, and following up on enrollment progress to ensure that the Veteran is enrolled in VA or community health care services
What You'll Bring to SupportWorks:
- Bachelor of Social Work or equivalent educational background in human/social services/healthcare preferred
- At least two years of experience working in the healthcare field or related setting that requires navigating and coordinating healthcare services and resources
- Experience with the Veterans Administration, community resources, homelessness and Veteran services strongly preferred
- Excellent computer skills including use of Microsoft Office Word and Excel
- Valid VA Driver's license and reliable transportation - you'll perform some local travel
