Location: Manchester, TN 37355 and surrounding service area
Pay: $56.14/hour plus mileage reimbursement at $0.41 per mile
Schedule: Monday-Friday, 8:00 AM-4:30 PM, with participation in on-call rotation
Employment Type: W2 Temp-to-hire, high potential for extension or conversion to full-time employment based on business needs
Job Summary
A healthcare organization specializing in hospice and home-based care services is seeking an experienced Hospice RN Case Manager to provide direct patient care and case management services throughout the Manchester, Tennessee service area. This role is responsible for managing a caseload of hospice patients, conducting assessments, coordinating care plans, collaborating with interdisciplinary teams, and supporting patients and families throughout the hospice care journey. The position requires daily patient visits within the assigned coverage area and participation in an established on-call rotation schedule.
Key Responsibilities
- Conduct comprehensive nursing assessments and ongoing evaluations of hospice patients
- Develop, implement, and update individualized care plans based on patient needs
- Manage approximately 21-25 patient visits per week throughout the assigned territory
- Coordinate care with physicians, social workers, discharge planners, and other healthcare professionals
- Provide patient and family education regarding disease progression, symptom management, and available resources
- Perform wound care and other skilled nursing interventions as needed
- Monitor patient status and adjust care plans accordingly
- Coordinate care transitions and facilitate access to appropriate healthcare and community resources
- Maintain accurate and timely clinical documentation within HomeCare HomeBase (HCHB)
- Participate in the organization's on-call rotation schedule
- Advocate for patients while maintaining confidentiality and privacy standards
- Identify opportunities for health promotion, illness prevention, and improved patient outcomes

Required Qualifications
- Active and unrestricted Registered Nurse (RN) license
- Hospice nursing experience required
- HomeCare HomeBase (HCHB) experience required
- Wound care knowledge and experience required
- Current CPR/BLS certification
- Valid driver's license
- Current automobile insurance
- Ability to travel throughout assigned coverage area for patient visits
- Strong assessment, care coordination, and clinical decision-making skills
Preferred Qualifications (Not Required)
- Associate's or Bachelor's Degree in Nursing
- Case Management Certification
- Experience coordinating care across multiple healthcare settings
- Knowledge of hospice regulations, documentation requirements, and interdisciplinary care planning
