(UHCS) provides RN-supervised, non-medical home care for seniors, adults with chronic conditions, and individuals recovering from illness or hospitalization across Fort Lauderdale and Broward County, Florida. The organization focuses on personalized care plans that are developed and monitored by a registered nurse and coordinated closely with each client’s physicians and medical team. Caregivers are bonded, insured, and covered under workers’ compensation, and UHCS ensures families have no payroll, tax, or liability responsibilities. Services include personal care, mobility assistance, fall prevention, medication reminders, meal preparation, companionship, and post-hospital recovery, all delivered with an emphasis on independence, dignity, and safety. UHCS also offers free in-home consultations and safety assessments to help clients and families make informed care decisions.\n
Role Description This is a part-time, on-site role for Physical Therapists and Physical Therapist Assistants located in Fort Lauderdale, FL. The role involves providing individualized physical therapy services in clients’ homes, including assessments, development of treatment plans, and delivery of therapeutic exercises and interventions to support mobility, functional independence, and recovery. Team members will collaborate with the supervising RN, physicians, and other care providers to coordinate care, monitor progress, and adjust plans as needed. Day-to-day responsibilities include documenting treatments, educating clients and family members on safe mobility and fall prevention, and promoting quality patient care in alignment with UHCS standards. Physical Therapists and Physical Therapist Assistants are expected to maintain professional communication, uphold safety protocols, and contribute to a respectful, client-centered environment.\n
Qualifications

  • Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement effective treatment plans.
  • \n

  • Demonstrated commitment to Patient Care and Quality Patient Care, focusing on safety, comfort, and functional outcomes.
  • \n

  • Experience or training in Orthopedics, including management of musculoskeletal conditions and post-surgical recovery.
  • \n

  • Current licensure or certification as a Physical Therapist or Physical Therapist Assistant in the state of Florida.
  • \n

  • Prior experience in home health, geriatric care, or community-based settings is highly beneficial.
  • \n

  • Ability to work independently in clients’ homes, with strong time management, documentation, and communication skills.
  • \n

  • Capacity to safely assist with mobility, transfers, and therapeutic exercises, including the ability to meet physical demands of the role.
  • \n

  • Reliable transportation to visit clients across Fort Lauderdale and Broward County.
