Key Responsibilities
- Provide skilled physical therapy services to patients in a home health setting
- Meet productivity expectations of 30 points per 40-hour work week
- Manage a patient caseload primarily within a central and mid-county territory
- Support additional service areas within the office radius as needed
- Document all patient care accurately and timely using HCHB electronic medical records
Required Qualifications
- Active and unrestricted Physical Therapist (PT) license in the state of Washington
- Minimum of 1 year of home health experience with a Medicare-certified facility
- Proficiency with HCHB (HomeCare HomeBase) EMR system required
- Ability to work a Monday–Friday daytime schedule with no on-call or weekend requirements
Preferred Experience / Skills
- Experience managing a multi-area home health territory
- Strong time management and self-directed work skills in a field-based environment
