Home Health Registered Nurse (RN) Contract
Location: Denver, CO
Schedule: Monday to Friday, 8:00 AM 5:00 PM
Duration: 13 Weeks
Hours: 40 hours/week (No weekends, No call)

Position Overview:
We are seeking a reliable and experienced Home Health RN to deliver high-quality care to patients in their homes throughout the Denver area. This contract role is ideal for an independent, organized nurse who thrives in a field-based setting and is comfortable with OASIS documentation.

Requirements:

• Active RN license in Colorado (or compact)

• Minimum of 1 year recent home health experience

• Strong proficiency in OASIS-E and timely 2448 hour documentation

• Familiarity with HomeCare HomeBase (HCHB) strongly preferred

• Valid drivers license and reliable transportation

• Excellent clinical judgment and communication skills

Responsibilities:

• Perform SOC, follow-up, discharge, and re-certification visits

• Assess and monitor patient health conditions and progress

• Collaborate with physicians and interdisciplinary team members

• Provide patient/caregiver education and coordinate care planning

• Complete accurate and timely documentation within agency timeframes
