Home Health PT - East Portland OR and Surrounding Areas

Job Details:
\n
• 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
\n• May visit up to 6 patients per day with 30 productivity points as standard
\n• 1:1 patient ratio
\n• No weekend requirement
\n• No on-call requirement
\n• Minimum 1 year of skilled home health experience required
\n• First-time travelers accepted
\n• Local candidates accepted within a 50-75 mile radius
\n• Facility may call off up to one shift per pay period (every two weeks)
\n• Traveler is expected to have their own transportation
\n• Occasional maximum travel radius of roughly 60 miles (60-90 minute total drive time) per day
\n• System groups clients to minimize drive and windshield time
\n• OASIS experience required
\n• Epic charting experience preferred (EPIC Remote Client for Home Health on laptop and Rover phone app)
\n• Business casual attire required; scrubs allowed but not required and no specific color required; no jeans
\n• Mileage reimbursement provided at IRS standard rate
\n• Free parking
\n• Required certifications: BLS
\n• Patient population includes adults and geriatrics
\n• Common patient diagnoses include dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke patients
