Salary Range: $60,312 -$74,755
Position Purpose:
The primary purpose of your job position is plan, develop, organize, implement, evaluate, and direct the Social Service Department in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, and as may be directed by the Administrator, to ensure that the medically related emotional and social needs of the individual receiving services are met/maintained on an individual basis. The Social Services Director oversees personnel working within the Social Services Department.

Organizational Responsibilities:
- Adhere to, support and promote St. Francis Health Services’ Mission and Core Values, philosophy, goals and objectives while conducting company business.
- Maintain confidentiality of company and individual receiving services related information at all times.
- Develop, maintain, and update as required, written policies and procedures that govern the day to day functions of the department.
- Serve on, participate in, and attend various committees of the care center as appointed by the Administrator.
- Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Director of Nursing or Administrator as required.
- Plan, develop, organize, implement, evaluate and direct the social service programs of this care center. Participate in planning and conducting educational in-service classes as directed.
- Provide consultation to members of staff, community agencies, etc., in efforts to solve the needs and problems of the individual receiving services through the development of social service programs.
- Evaluate and implement recommendations from established committees as they may pertain to social services.
- Participate in community planning related to the interests of the care center and the services and needs of the individual receiving services and family.
- Maintain contact and positive working relations with referral sources who are regularly updated on openings for admission.
- Receive admission inquiries, provide information on the care center and services provided, maintain care center waiting list.
- Serve as discharge planning coordinator, develop and implement social care plans and individual receiving services assessments as needed.
Individual Receiving Services Care Responsibilities:
- Does not disclose individual receiving services’ protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
- Ensure that all care is provided in privacy, as appropriate, and to knock after entering an individual receiving services’ room.
- Report all grievances and complaints made by the individual receiving services, family, or visitors to the appropriate supervisor following the care center’s established procedures.
- Orient individual receiving services to the care center and its policies; assist with review and completion of the Individual receiving services Bill of Rights, admission agreement and other admission documents.
- Assist individual receiving services and families with adjustment to the care center through regular contact.
- Evaluate social and family information of the individual receiving services and participate in the process of developing, implementing, and updating a comprehensive plan of care and assessment for each individual receiving services, timely providing all needed social service information and perform charting duties as required.
- Ensure that all individual receiving services are provided the opportunity to participate in the development and review of their plan of care.
- Provide medically related social services to assure that each individual receiving services' optimal level of functioning and needs are met.
- Provide individual receiving services with information concerning individual receiving services rights, living wills, etc. Explain as necessary.
Individual Responsibilities:
- Report all complaints and grievances, according to company policies and procedures, which are made by the individual receiving services, family, or visitors and make a written/oral report to supervisor indicating what action(s) were taken to resolve the complaint or grievance.
- Report any communicable or infectious disease that you contract to the appropriate supervisor.
- Develop and maintain a good working rapport with department personnel, other departments within the care center, and outside community health, welfare and social agencies, to assure that social service programs can be properly maintained and coordinated to meet the needs of the individual receiving services.
- Represent the care center through involvement in community, civic, or professional organizations as appropriate.
Personnel Responsibilities:
- Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken. Follow care center’s established procedures.
- Recruit, interview, and select personnel for the department.
- Determine departmental staffing requirements necessary to meet the department’s needs, and assign a sufficient number of personnel for each tour of duty.
- Schedule department work hours (including vacation and holiday schedules), personnel and work assignments.
- Counsel/discipline department personnel as necessary.
- Terminate employment of personnel when necessary, documenting and coordinating such actions with the HR Director and/or Administrator.
- Conduct departmental performance evaluations in accordance with the care center’s policy and procedures.
- Develop and participate in the planning, conducting, and scheduling of timely in-service training classes that provide instructions on “how to do the job” and ensures a well-educated department.
- Ensure that all department personnel attend and participate in care center mandatory in-service training programs as scheduled (e.g. OSHA, TB, HIPAA, Abuse Prevention, etc.).
- Develop, maintain, and periodically update the written procedure for ensuring that department personnel maintain valid and current credentials as may be required.
- Coordinate the training of social services’ personnel.
- Ensure that all department personnel are knowledgeable of the individual receiving services’ rights and responsibilities, including the right of refusal.
- Coordinate evaluations of performance and meet with all social services personnel at least annually with the intent of promoting job growth, development, and improvement of services as needed.
- Delegate and monitor responsibilities to departmental staff consistent with their training, experience, competence and legal authorization in accordance with care center policy.
- Complete MDS Sections C, D, E, Q, and V
- Complete BIMS, PHQ-9, Social Determinants, Vulnerability Assessment, Discharge Planning Assessment, and Trauma-Informed Care Assessment
- Coordinates all discharge processes including home health, DME, transportation, education, and safe transition planning.
- Completes required discharge assessments (BIMS, PHQ-9, Satisfaction Survey)
- Provides and documents Notice of Discharge/Transfer and coordinates monthly Ombudsman reporting.
- Serves as primary backup to the Admissions Nurse
- Screens and reviews referrals, communicates with referral sources, and forwards to nursing for clinical approval.
- Completes Admission paperwork and PCC setup when Admission Nurse is absent.
- Serves as the facility’s designated Grievance Official
- Investigates, documents, resolves, and tracks all grievances
- Maintains grievance log and reports monthly findings at QAPI
- Coordinates ACP mental health services
- Provides crisis intervention, grief support, and trauma-informed psychosocial care
- Integrates behavioral and trauma-informed interventions into care plans
- Assists with OHFC investigations and follow-up as directed by DON
- Completes MAARC reports as indicated
- Ensures PASSAR/OBRA Level II documentation is complete prior to admission
- Assists residents and families with Medical Assistance (MA) applications, insurance-related forms, and financial guidance
- Helps interpret insurance coverage, appeals, denials, and LTC financial requirements
- Acts as a primary advocate for residents and families navigating complex healthcare systems
- Provides 1:1 psychosocial/emotional support
- Supports residents experiencing grief, depression, anxiety, mental anguish, or adjustment concerns
- Assists with end-of-life planning and hospice coordination
