As a Licensed Clinical Social Worker (LCSW) with Complete Well-Care Source, LLC (CWCS), you will provide professional mental health services, including assessment, diagnosis, treatment, case management, and coordination of care. You will work with individuals, families, and groups, ensuring services align with CWCS’s mission of enhancing daily living through person-centered, recovery-focused care.
Essential Duties & Responsibilities
- Clinical Assessment & Diagnosis
- Conduct comprehensive biopsychosocial evaluations.
- Develop individualized treatment plans.
- Diagnose mental health disorders per DSM-5 criteria.
- Therapeutic Interventions
- Provide evidence-based therapy (CBT, DBT, trauma-informed care, etc.).
- Offer crisis intervention and risk assessments.
- Facilitate peer support and recovery-focused interventions.
- Case Management & Client Advocacy
- Coordinate care with healthcare providers and community resources.
- Advocate for client rights and Medicaid-funded services.
- Assist clients with accessing housing, employment, and support programs.
- Compliance & Documentation
- Maintain accurate, confidential client records (HIPAA & NC Medicaid compliance).
- Complete timely clinical documentation, including progress notes and treatment plans.
- Participate in audits and quality assurance reviews.
- Collaboration & Professional Development
- Work closely with Qualified Professionals (QPs), Peer Support Specialists (CPSS), and the CWCS team.
- Attend case staffing meetings, treatment teams, and supervision sessions.
- Stay updated on industry best practices and required trainings.
EMPLOYMENT TERMS & CONDITIONS
- Compliance & Professional Standards
- Maintain active LCSW licensure in North Carolina.
- Follow HIPAA, Medicaid regulations, and CWCS policies.
- Participate in ongoing training and supervision.
- Confidentiality & Ethics
- Adhere to ethical standards in client care.
- Maintain strict confidentiality of client information.
- Report any ethical or legal concerns to supervisors.
