Job Title: Licensed Practical Nurse (LPN) – Full-Time
Schedule:
- 12-hour shifts
- Full-time, 6:00 AM – 6:30 PM
- 40 hours per week
Pay: $44.45/hr
Job Summary:
We’re seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. The ideal candidate will provide quality nursing care to residents while ensuring their comfort, safety, and well-being. This position involves administering medications, performing treatments, and collaborating closely with the interdisciplinary care team.
Responsibilities:
- Administer medications and perform physician-ordered treatments within scope of practice (oral, enteral, parenteral, suppository, wound care, tracheostomy, gastrostomy, catheter care, etc.)
- Observe, document, and report the effectiveness of medications and treatments
- Ensure adequate inventory of medical supplies and medications
- Participate in shift change reports and communicate effectively with residents, families, and staff
- Complete initial admission assessments and contribute to care plans and quarterly MDS reviews
- Maintain accurate clinical and administrative documentation
- Support and participate in Quality Assurance and Performance Improvement (QAPI) activities
- Attend and actively engage in staff meetings
Knowledge & Skills:
- Geriatric and gerontological nursing theory and practice
- Understanding of the aging process and related healthcare needs
- Principles of pharmacology, medication administration, and secure handling of medical supplies
- Ability to observe, assess, and report resident conditions accurately
- Strong teamwork and communication skills — both verbal and written
- Experience assisting in developing and implementing patient care plans
Requirements:
- Level one Fingerprint clearance card
- CPR BLS
- TB within six months of hire
- AZ LPN license
- Covid Vaccine & Hep B or declination for both
- Pass a Background check and drug screen
