We are looking for a dedicated Case Manager to join our team in San Francisco, California. This position involves working closely with formerly homeless individuals, providing essential support and case management services to help them maintain stable housing and improve their quality of life. As a Long-term Contract role, this opportunity is ideal for someone passionate about advocacy and community engagement.
Responsibilities:
• Manage a caseload of 60-90 units, providing tailored support to individuals with diverse challenges, including mental health, substance abuse, and medical issues.
• Conduct outreach visits upon tenant entry, during follow-up periods, and as needed to ensure consistent engagement.
• Perform comprehensive needs assessments and deliver ongoing case management services focused on housing retention and life improvement.
• Assist tenants in maintaining housing by fostering strong relationships, offering guidance, and implementing effective intervention strategies.
• Support tenants in accessing benefits, managing rent payments, and addressing unit habitability concerns.
• Coordinate monthly pest control and unit inspections alongside hotel management staff.
• Organize community-building activities such as tenant groups, social events, and collaborative programs to foster a supportive environment.
• Refer tenants to appropriate services, including job placement programs, and ensure successful follow-through.
• Respond promptly to tenant crises, providing de-escalation techniques and immediate support.
• Maintain accurate and confidential case management files and documentation, ensuring compliance with organizational standards.

• Proven experience in administrative assistance, data entry, and filing.
• Strong interpersonal skills and the ability to build rapport with diverse populations.
• Familiarity with tenant case management practices, including needs assessments and intervention strategies.
• Ability to manage a high-volume caseload while maintaining attention to detail.
• Knowledge of community resources and experience connecting individuals to appropriate services.
• Excellent crisis management and problem-solving skills.
• Proficiency in maintaining confidentiality and adhering to ethical guidelines.
• Availability to attend scheduled meetings and collaborate effectively with internal teams and external providers.
