- Salary Range: $24.04-$28.85 per hour, based on relevant years of experience
- Location: Fort Collins and Loveland, Colorado
- Status: Full time, 40 hours per week
- Hours: Monday – Friday 8:00AM to 5:00PM
Role Overview:
Responsible for delivering community-based care coordination services to youth and families experiencing behavioral health challenges by assessing needs and linking clients to behavioral health, substance use, medical, and community resources. This role plays an essential part in the Behavioral Health Referral Team partnership between SummitStone Health Partners, the Health District of Northern Larimer County, and Poudre School District. The Care Coordinator collaborates across multidisciplinary and multi‑agency teams to ensure seamless support and navigation for clients throughout Larimer County.
Essential Duties:
- Provide comprehensive care coordination services in partnership with clients and their support systems.
- Conduct outreach through referral pathways, community partners, and agency‑led engagement efforts.
- Coordinate care with a broad range of community partners, including emergency departments, primary care providers, acute care systems, school districts, crisis response teams, and behavioral health providers.
- Support clients in navigating complex systems of care and improving continuity of services.
- Participate in multidisciplinary teams and provide support and resources within the scope of the position.
- Collaborate with SummitStone staff and the Colorado Opioid Synergy Larimer and Weld (CO-SLAW) network.
Required:
- Bachelor’s degree in psychology, social work or related field from accredited College or University. Equivalent combinations of education and experience may be considered.
- At least one year of experience in care coordination or case management within the mental health field, substance use disorder field, or program area required.
- Experience supporting individuals with mild to severe mental illness and substance use disorders.
- Experience navigating systems of care.
- Ability to travel to agency and community locations.
Licenses and Certifications:
- Valid Colorado driver’s license or the ability to obtain one within 30 days of hire required; must maintain a clean driving record. This role requires occasional travel to client homes and community partner sites.
- CAT, CAS, CAC III, LAC or willingness to pursue preferred
Highly Desired:
- Experience supporting youth and/or family systems
- Bilingual/bi-cultural
