Job DescriptionJob Opportunity: Occupational Therapist

Our organization is in search of a dedicated and empathetic Occupational Therapist to become a vital member of our Healthcare & Medical Services team. In this role, you will deliver exceptional, evidence-based therapy services to patients requiring rehabilitation and assistance in achieving their optimal level of functional independence.

Key Responsibilities

  • Conduct thorough evaluations and create personalized treatment plans.
  • Implement therapeutic interventions aimed at enhancing daily living activities, functional mobility, cognitive abilities, and safety in both home and community settings.
  • Work collaboratively with a multidisciplinary care team, including nursing staff, physical and speech therapists, physicians, social workers, and case managers, to provide coordinated and comprehensive care.
  • Educate patients, families, and caregivers on adaptive strategies, the use of assistive devices, energy conservation techniques, and home safety modifications to foster long-term success and minimize the risk of rehospitalization.
  • Accurately document all assessments, treatment sessions, progress notes, and discharge plans in the electronic medical record, adhering to state, federal, and organizational guidelines.

Working Environment

This position typically operates in a hybrid setting, which may include skilled nursing facilities, rehabilitation centers, and community or home health environments, depending on patient requirements and service lines. Flexibility, dependability, and the ability to work autonomously as well as part of a collaborative team are essential.

Qualifications

  • Must be a licensed Occupational Therapist.
  • Strong clinical reasoning and excellent communication skills are required.
  • A genuine passion for working with diverse patient populations, particularly older adults and those with complex medical and functional needs.
  • Ability to manage a caseload effectively, prioritize patient needs, and adjust treatment plans as necessary.
  • Commitment to evidence-based practices, ethical care, and compliance with regulations is crucial.
