About the Role

The Clinical Care Coordinator is a cornerstone of our clinical operations and a powerful advocate for the clients we serve. This role sits at the intersection of clinical quality, coordination, and continuity of care, ensuring that every client’s treatment journey is intentional, compliant, and deeply human.

You ensure that treatment plans are aligned, documentation tells the client’s story accurately, and transitions between levels of care are thoughtful and successful. What You’ll Do

As a Clinical Care Coordinator at PHRI, you will:

  • Serve as a clinical coordination leader for inpatient and residential SUD treatment programs, ensuring seamless care from admission through discharge.

  • Oversee and monitor treatment planning, diagnostic assessments, utilization reviews, and discharge coordination under clinical supervision.

  • Act as a primary liaison with managed care organizations, DBH, DHCF, and other payors, ensuring services are authorized, compliant, and timely.

  • Track and monitor treatment plans, utilization data, and status reports across EHR systems (including Credible), ensuring accuracy and readiness.

  • Collaborate closely with clinical staff, medical providers, case managers, billing teams, and external agencies to align care and documentation.

  • Ensure clinical documentation meets regulatory, ethical, and accreditation standards, including HIPAA and 42 CFR requirements.

  • Support psychiatric service coordination on-site, providing administrative and clinical monitoring support.

  • Identify concerns early and communicate proactively with the Clinical Director/Supervisor to mitigate risks and improve care delivery.

  • Assist with staff onboarding, training, meetings, in-service sessions, and compliance-related activities.

  • Contribute to the development and refinement of coordination structures that adapt to real-world operational needs.

What You’ll Bring

You bring both clinical expertise and systems-level thinking to the table. Specifically, you have:

  • Independent clinical licensure (LPC or LICSW) in good standing, with approved supervisor certification if applicable.

  • A Master’s degree in Social Work, Psychology, Counseling, or a related behavioral health field.

  • 3–5 years of experience in community-based behavioral health settings.

  • Strong familiarity with DC behavioral health systems, community resources, and government agencies.

  • Working knowledge of medical terminology, ICD/CPT coding, utilization management, and regulatory standards.

  • Experience with EHR systems such as Credible, GAINS, LOCUS, or ICAMs.

  • Excellent organizational, communication, and critical-thinking skills.

  • The ability to apply sound judgment in both structured and emergent situations.
