Manager, Case Management
Job Summary
The Manager of Case Management provides leadership and operational oversight for hospital-based case management services within an assigned area of responsibility. Reporting to the Case Management Director, this role promotes patient-centered care by coordinating all aspects of case management in alignment with departmental and organizational goals.
The Manager serves as a clinical expert and subject matter resource, ensuring effective care coordination, appropriate utilization of resources, regulatory compliance, and high-quality documentation. This position is accountable for the daily management, supervision, and development of the case management team while supporting quality outcomes and efficient patient throughput.
Key Responsibilities
- Provide day-to-day leadership and administrative oversight of the case management program.
- Supervise, coach, and support professional and support staff to ensure consistent, high-quality case management practices.
- Serve as a clinical expert and resource for case managers, offering guidance on complex cases and care coordination strategies.
- Ensure compliance with hospital policies, regulatory requirements, and applicable standards and guidelines.
- Monitor quality of case management documentation, assessments, and interdisciplinary communication.
- Promote effective patient throughput while balancing optimal care delivery and appropriate resource utilization.
- Coordinate quality improvement initiatives that enhance patient outcomes and operational performance.
- Collaborate with interdisciplinary teams, physicians, and leadership to support continuity of care and discharge planning.
Qualifications
Education
- Bachelor’s Degree in Nursing OR
- Licensed Clinical Social Worker (LCSW) with a Master’s Degree in Social Work – Required
Experience
- Minimum of 3 years of clinical nursing experience in an acute care setting OR hospital social work experience – Required
- Minimum of 2 years of supervisory or leadership experience in an acute care setting – Required
Licensure
- Current, unrestricted Registered Nurse (RN) license OR
- Current LCSW or LMSW license in the applicable state(s) – Required
Preferred Qualifications
- Experience using InterQual criteria
- Certification in Case Management (CCM, ACM) and/or InterQual or IQCI certification
