We are seeking an experienced Registered Nurse (RN) Case Manager for Penn Medicine Home Health – Princeton Health (South Team). The ideal candidate will have a strong background in home health, case management, and OASIS documentation, with the ability to manage a high-volume patient caseload efficiently.
Key Responsibilities:

Manage and coordinate care for home health patients

Perform patient assessments and develop individualized care plans

Complete accurate and timely OASIS documentation
-
Handle a high patient volume of 5–6 visits per day
-
Ensure proper documentation and compliance with healthcare standards
-
Provide patient and family education on care plans

Collaborate with physicians and interdisciplinary teams
-
Perform wound care (preferred) and monitor patient progress
Requirements:
-
Active Registered Nurse (RN) license
-
Minimum 2+ years of Home Health experience

Strong Case Management experience (required)

Mandatory experience with OASIS documentation

Experience in a high-volume home health setting

Wound care experience preferred

Excellent organizational and documentation skills
Work Environment:

Field-based role covering Plainsboro and West Windsor, NJ
