Job Title: Patient Care Coordinator

Position Summary:
A Patient Care Coordinator serves as the primary point of contact for our office patients and our nursin g home. patients, helping coordinate healthcare services, appointments, and communication between patients, providers, and insurance companies. The role focuses on ensuring patients receive timely, efficient, and compassionate care throughout their healthcare journey.

Key Responsibilities

  • Schedule, confirm, and coordinate patient appointments.
  • Serve as the primary liaison between patients, families, healthcare providers, nursing facilities, and insurance companies.
  • Verify insurance eligibility and obtain prior authorizations when required.
  • Maintain accurate and confidential patient records in accordance with HIPAA regulations.
  • Enter billing and patient information accurately into the billing system.
  • Answer incoming telephone calls professionally, direct calls as needed, and take detailed, accurate messages.
  • Perform data entry into specialized scheduling software to coordinate visits for nursing home patients.
  • Ensure all patient information is current and complete to facilitate timely care.
  • Assist with general administrative duties and support the clinical team as needed.
  • Communicate effectively with nursing facilities, physicians, and staff to ensure continuity of patient care.

Qualifications and Skills

  • Previous experience in a medical office, healthcare, or patient scheduling environment preferred.
  • Strong organizational and time-management skills with the ability to prioritize multiple tasks.
  • Self-motivated and able to work independently with minimal supervision.
  • Exceptional attention to detail and commitment to accuracy.
  • Excellent verbal and written communication skills.
  • Proficient in data entry and comfortable learning specialized healthcare software.
  • Professional telephone etiquette and outstanding customer service skills.
  • Knowledge of medical terminology, insurance verification, and billing procedures is a plus.
  • Ability to maintain confidentiality and comply with HIPAA requirements.
  • Proficiency with Microsoft Office (Word, Outlook, and Excel) preferred.
