Job Summary:

The Intake Coordinator is responsible for managing the intake and assessment process for new members, ensuring their needs are accurately identified and addressed. This role involves coordinating with various stakeholders, maintaining accurate records, and supporting both members and care managers in achieving their goals. The Intake Coordinator also plays a crucial role in data management, compliance, and communication to ensure efficient and effective service delivery.

Responsibilities:

  • Conduct member intake and assessments to determine placement requirements.
  • Identify necessary housing, health services, and non-housing supports to develop member action plans.
  • Assist and follow up with Enhanced Care Managers and members regarding resource and referral information.
  • Collaborate with management for member updates and case assignments to manage and monitor members' progress and goals.
  • Update member files upon intake completion, and prepare documentation and reports on intake numbers.
  • Utilize provider portals to verify and update member status and information.
  • Maintain up-to-date knowledge of company requirements and platforms.
  • Prepare for and participate in weekly meetings to update team members on intake numbers, member statuses, and policy changes.
  • Build and maintain professional relationships with multiple providers.
  • Mediate between partnered health plans and their members.
  • Make intake calls as assigned and collaborate with the Operations Manager to reach team goals.
  • Perform additional duties as assigned.

Education and Experience:

  • High school, diploma or GED is required.
  • Previous experience in case management, social work, or healthcare coordination preferred.
  • Previous experience handling multiple clients over the phone preferred.

Required Skills/Abilities:

  • Excellent communication, interpersonal, customer service and organizational skills.
  • Proficiency in multitasking, organization, and attention to detail.
  • Ability to work in a team environment and independently.
  • Proficiency in Microsoft Office Suite, data entry and electronic systems.
  • Computer skills for documentation, email and communications.
  • Bilingual in Spanish preferred.

Physical Requirements:

  • Prolonged periods of sitting at an office desk on the computer.
  • Lifting: Able to lift up to 15lbs.

Pay range$20—$25 USD
