Description of the role:
University Home Care is currently seeking Physical Therapists for home care positions in Wayne, Oakland, or Macomb counties. We need full time, part time and contingents please. As a Physical Therapist, you will be responsible for providing direct patient care in accordance with established plans and treatments. This role requires strong communication skills and the ability to work independently and collaboratively with a multidisciplinary team.
Responsibilities:
- Conduct initial evaluations and assessments of patients' conditions
- Develop and implement individualized treatment plans
- Monitor patients' progress and adjust treatment plans accordingly
- Educate patients and their families on home exercise programs and adaptive techniques
- Assist patients with activities of daily living and functional mobility
- Document patient progress and maintain accurate medical records
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care
Requirements:
- Valid and current Physical Therapy license in the state of Michigan
- Previous experience in home care is preferred
- Strong knowledge of anatomy, physiology, and kinesiology
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Flexibility to travel within Wayne, Oakland, or Macomb counties
