This position involves working directly on-site, where you will assess and provide treatment for residents in various care environments, including assisted living, rehabilitation, and memory care.

Key Responsibilities

• Conduct evaluations and develop treatment plans tailored to individual resident needs.
• Focus on enhancing mobility, strength, balance, and managing pain effectively.
• Document patient progress and treatment details in the Electronic Medical Record (EMR) system.
• Work collaboratively with a multidisciplinary team to ensure comprehensive care.

Qualifications and Requirements

• Valid Physical Therapy license.
