Compensation:
$48 - $53 hourly
Responsibilities:
- Perform comprehensive assessments of patients’ medical conditions, symptoms, and pain levels to guide care decisions.
- Develop individualized care plans in collaboration with the interdisciplinary team and the patient’s loved ones.
- Continuously review and update care plans to reflect the patient’s evolving needs and ensure the highest quality of care.
- Monitor vital signs, treatment responses, and overall patient well-being, ensuring comfort and safety at every stage.
- Work closely with physicians, palliative care specialists, and other team members to optimize pain relief and symptom management.
- Provide compassionate, emotional, and spiritual support to patients and families, recognizing the unique challenges of end-of-life transitions.
- Honor patients’ cultural and spiritual values, fostering an environment of respect, dignity, and peace.
- Lead and participate in interdisciplinary team meetings to align on care goals and ensure seamless communication.
- Coordinate care among hospice staff, including social workers, chaplains, home health aides, and volunteers.
- Serve as a liaison with external healthcare providers and community organizations to ensure patients have access to all available support systems.
- Educate patients and families about the hospice philosophy, care journey, and available services.
- Offer expert guidance on pain management and coping strategies for both patients and caregivers.
- Maintain timely, accurate documentation of all patient care, interactions, and updates to ensure continuity and regulatory compliance.
Qualifications:
- Must hold an active Registered Nurse (RN) license in the state of New Jersey.
- A minimum of 2–3 years of clinical nursing experience is required, with strong preference for those with backgrounds in hospice, palliative care, or related specialties.
- Deep understanding of hospice philosophy and the core principles of end-of-life care is essential.
- Exceptional communication and interpersonal skills to effectively engage with patients, families, and members of the interdisciplinary team.
- Demonstrated empathy, compassion, and emotional sensitivity in providing support through serious illness, grief, and loss.
- Ability to work independently in the field while remaining a reliable and collaborative member of the care team.
- Strong organizational and time-management abilities to balance multiple patient needs and documentation requirements.
- Comfortable using electronic health records (EHRs) and relevant healthcare software to document care accurately and efficiently.
