20-30+ Hours Per Week

$22-$25/hour, depending on experience
Are you an exceptional caregiver with experience supporting individuals living with Alzheimer's disease, dementia, Parkinson's disease, or other complex aging-related conditions?Griswold Home Care for Lakewood & Golden is seeking a compassionate, professional, and experienced Lead Caregiver to help us deliver exceptional care to seniors throughout Jefferson County.This is more than a caregiving position. As our Lead Caregiver & Client Support Specialist, you will serve as a trusted resource for clients, families, caregivers, and the office team. You will help mentor caregivers, support families, participate in client assessments, and play a key role in maintaining the high standard of care our clients deserve.

Schedule & Coverage Support

  • Provide direct client care as needed
  • Assist with difficult-to-fill shifts within designated availability
  • Support continuity of care during schedule changes and caregiver absences
  • Help evaluate caregiver-client compatibility and recommend staffing adjustments

Client & Family Support

  • Conduct client introductions and caregiver transitions
  • Participate in client assessments and 90-day supervisory visits
  • Assist with care plan development and updates
  • Communicate changes in client condition to the office and family
  • Provide family education and support regarding dementia care and aging-related challenges
  • Build strong relationships with clients and families
  • Help resolve concerns and ensure a positive client experience

Caregiver Support & Mentorship

  • Serve as a resource for caregivers needing guidance and support
  • Assist caregivers with dementia care strategies and challenging behaviors
  • Coach caregivers on transfers, mobility assistance, personal care, and safety techniques
  • Support new caregivers during onboarding and their first assignments
  • Review care plans and client expectations with new staff
  • Conduct caregiver check-ins and mentorship visits
  • Help identify caregiver burnout and provide encouragement and support

Quality Assurance

  • Conduct quality assurance visits with clients and caregivers
  • Monitor client satisfaction and caregiver performance
  • Ensure care plans are being followed appropriately
  • Assist with incident follow-up and problem resolution

Dementia Care Leadership

  • Serve as the agency's dementia care resource
  • Provide dementia behavior coaching and caregiver training
  • Develop strategies for wandering, agitation, sundowning, refusal of care, and safety concerns
  • Support families navigating cognitive decline and changing care needs
  • Assist with dementia-focused programs and initiatives

Community Outreach

  • Attend networking and community events
  • Represent Griswold professionally in the community
  • Deliver marketing materials and thank-you gifts to referral partners
  • Participate in educational presentations and outreach activities
  • Assist with local marketing and referral development efforts

Qualifications

  • Experience caring for individuals with Alzheimer's disease, dementia, and related conditions
  • Excellent communication and interpersonal skills
  • Professional, dependable, and compassionate demeanor
  • Strong problem-solving skills and ability to work independently
  • Reliable transportation and willingness to travel throughout Jefferson County
  • Previous leadership, training, mentoring, or supervisory experience preferred
