Senior Community Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.

Pay: $58-$70k annually

Schedule: Monday - Friday 8:00am - 5:00pm, occasional weekends.

The Personal Services Supervisor will supervise all direct staff in the Adult Day Health Center and the Home Care departments in conformance with policies, procedures, and standards and all governing state and federal regulations. Responsibilities include the planning, coordination, and implementation of the dementia programming for all direct care staff.

QUALIFICATIONS:

  • Bachelor’s Degree or equivalent education and experience.
  • Current certification with appropriate state in which program operates as Certified Nursing Assistant (C.N.A.).
  • Three (3) years’ experience in a geriatric health setting including a minimum of two (2) years of demonstrated successful supervisory experience. Management experience preferred.
  • Certification or equivalent experience demonstrating knowledge of comprehensive dementia care.
  • Proficiency with various business software, preferably Microsoft Office.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must have medical clearance for communicable diseases and up-to-date immunization after having direct participant contact.

Skills and Knowledge:

  • Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.).
  • Working knowledge of the PACE regulations and state regulations.
  • Working knowledge of healthcare and aging networks.
  • Working knowledge of physical, mental, spiritual, and social needs of the frail elderly and their families.
  • Working knowledge of quality improvement and cost containment systems.
  • Demonstrated effective leadership and decision making skills.
  • Demonstrated strong facilitation and conflict resolution skills.
  • Ability to foster and build professional relationships among staff.
  • Ability to effectively and efficiently plan, prioritize and follow-up on/delegate responsibilities.
  • Effective written and oral communication skills.
  • Working knowledge of local health care and geriatric service networks.

ESSENTIAL FUNCTIONS:

  • Directs, supervises and evaluates the performance of the C.N.A.s, and P.C.A.s. Completes supervisory visits within scope and in conjunction with Manager of Home Care.
  • In consultation with the Manager of Home Care, Day Center Supervisor and other management staff, assists in developments and implementation of policies, procedures and standards for the Adult Day Health Center and Home Care departments. Communicates changes to staff, monitors outcomes and makes changes as needed.
  • Conducts interviews and recommends candidates for hire.
  • Provides for orientation, ongoing training and competency evaluation of staff according to program needs, and state and federal regulatory requirements. Ensures that the job responsibilities, authorities and accountabilities of all direct reports are defined and understood.
  • Fosters team-building, professional relationships and good communication among staff and other departments. Routinely communicates and implements changes in program goals, philosophy and policy to staff.
  • Identifies the continuing education needs of assigned staff and collaborates with the Center Director and Director of Clinical Services and Quality and Compliance Manager to establish the most effective and efficient approaches to addressing same.
  • Assists in the development of organizational staffing patterns and position descriptions for assigned staff.
  • In conjunction with Manager of Home Care and Home Care Scheduler, establishes and maintains operational systems to ensure uniform and consistent delivery of care and services provided in the homes of participants.
  • Provides on-going support and training to all staff involved in dementia-specific programming related to the dementia disease process
  • In conjunction with Day Center Supervisor, establishes and maintains operational systems to ensure uniform and consistent delivery of care and services for participants in the Adult Day Health Center.
  • Works with the management staff and administration to assure that cultural needs of participants are met through services provided by personal care staff, whether in the homes of participants or in the Adult Day Health Center.
  • In consultation with Registered Dietitian and Day Center Supervisor, ensures staff are trained appropriately to maintain compliance and standards of food service and safety.
  • In conjunction with the management team, is responsible for maintaining compliance with all State and Federal program requirements, obtaining and maintain certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.
  • Functions as a member of the Interdisciplinary Team as a representative for Personal Care Attendant. Maintains regular attendance at, and participants in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour
    care delivery.
  • As a member of the Interdisciplinary Team, actively communicates relevant issues to the team and ensuring all team members are informed and up to date; acts as facilitator, when designated, of Interdisciplinary Team Plan of Care meetings.
  • Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
  • Acts within the scope of his or her authority to practice
  • Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Performs related duties as required or requested.
