Location: Bronx, New York
Duration: 13-Week Contract (High Possibility of Extension)
Shift: Day Shift, 5x7 Schedule
Guaranteed Hours: 35 Hours/Week

Compensation

Pay Rate: $25.00 - $30.00 per Hour

Position Overview

We are seeking a motivated and experienced Project Assistant (Social Determinants of Health Program Manager) to support and enhance initiatives focused on identifying and addressing patients' social needs. This role will serve as a key resource for social needs screening and referral programs, working closely with healthcare teams, community organizations, volunteers, and leadership to improve patient access to essential services and community resources.

The ideal candidate will have a strong background in public health, data analysis, program coordination, and stakeholder engagement, along with the ability to oversee projects and support workforce development initiatives.

Responsibilities

  • Lead and improve efforts across multiple departments to identify patients with unmet social needs and connect them with appropriate community resources.
  • Serve as a subject matter expert on Social Determinants of Health (SDOH) screening and referral processes.
  • Develop, implement, and expand workflows that support social needs screening and referral initiatives.
  • Collaborate with hospital staff, community-based organizations, and program stakeholders to evaluate outcomes and identify additional support resources.
  • Recruit, train, and supervise Health Advocates, including volunteers and interns.
  • Plan and facilitate comprehensive training programs, including recurring multi-day training sessions.
  • Support and train healthcare staff involved in social needs screening and referral activities.
  • Collect, analyze, and interpret program data to evaluate performance and identify opportunities for improvement.
  • Prepare presentations, reports, and program updates for leadership and stakeholders.
  • Provide administrative and operational support to ensure efficient program management and department operations.

Required QualificationsEducation

  • Bachelor's Degree required.

Experience

  • Minimum of 3 years of relevant professional experience required.
  • Experience in public health, community health, healthcare programs, or related fields preferred.
  • Supervisory experience preferred.

Required Skills

  • Strong knowledge of public health, health policy, community health, and social service systems.
  • Experience with data analysis, reporting, and program evaluation.
  • Proficiency with Microsoft Excel and other data management tools.
  • Ability to create presentations, reports, and program documentation.
  • Strong organizational, communication, and project management skills.
  • Ability to supervise, mentor, and support volunteers and interns.
  • Excellent problem-solving and stakeholder engagement abilities.
