A skilled nursing facility serving the eastern region of Massachusetts is seeking an experienced RN Medication Management Nurse for a full-time position. This is a non-bedside role with no clinical on-call, offering $105,000-$110,000 annually.

This position focuses on medication coordination, including therapeutic interchange processes, and collaboration with pharmacy providers and physicians to ensure safe, efficient, and compliant medication management for residents.

Key Responsibilities

• Review and ensure medications align with facility formulary and established protocols
• Coordinate therapeutic interchange processes in collaboration with pharmacy and physicians
• Facilitate prior authorizations and support medication adjustments
• Communicate medication updates with the interdisciplinary care team
• Support compliance with state and federal medication-related regulations
• Maintain accurate and timely documentation

Qualifications

• Active RN license required
• Experience in skilled nursing, post-acute care, or medication coordination preferred
• Strong organizational and communication skills
• Detail-oriented with the ability to manage multiple workflows
