Job Details:

• Work day shifts from 8:30 AM to 5:00 PM
\n• Typically rotate Saturdays once a month
\n• Minimum of 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting required
\n• Required certification: BLS
\n• Service area covers Yolo County including Woodland, Davis, Yolo, Dunnigan, Arbuckle, West Sacramento, Natomas, Knights Landing, Capay, Vacaville, Winters, and Esparto
\n• Charting system: Homecare Homebase (HCHB)
\n• Dress code: Scrubs optional; if not wearing scrubs, slacks (khakis acceptable) and a polo or button-up shirt are required; any color scrubs are acceptable
\n• Assess, evaluate, and update therapeutic, rehabilitative, and functional status of patients
\n• Evaluate home environments and assess muscle strength, mobility, gait, and range of motion
\n• Direct physical therapy treatments and determine priority needs for patients
\n• Instruct patients, families, and caregivers in the use and care of therapeutic appliances
\n• Report patient reactions to treatment or changes in condition to the attending physician
\n• Initiate physical therapy programs and instruct other personnel and family members in therapy phases
\n• May train patients in the use of prosthetic devices
\n• Supervise homemakers and Home Health Aides participating in patient rehabilitation regimens
\n• Identify patient and family needs for additional home health services and refer as necessary
\n• Prepare and submit clinical and progress summaries based on goal attainment
\n• Participate in peer consultation process and ongoing staff competency assessments
\n• Must have good oral and written communication skills and strong organizational skills
\n• 2 supervisory references from the submitting modality required at submittal (1 within 12 months and 1 within 3 years)
