Position Summary

Responsible for assessment and counseling and coordination of community services/referrals. Acts as an advocate for patients in the BJHCHS, Inc. health system. Maintains pertinent, accurate, timely, documentation in clinical record.


Essential Duties and Responsibilities

  • Perform accurate assessment and treatment planning through evaluation of clinical symptoms, development and personality disorders, psychosocial stressors, and global function
  • Provide effective counseling/case management services to patients
  • Coordinate care of patients with healthcare providers, community agencies, specialists
  • Coordinate and plans with outside agencies and home care nursing staff for post-discharge care
  • Maintain timely, accurate documentation on all medical records in compliance with established guidelines and policies
  • Demonstrate emotional resilience and stability
  • Demonstrate excellent communication skills with patients and staff
  • Capable of working with children and adults
  • Perform other related duties as assigned by management
  • Travel to various BJHCHS, Inc. medical center locations as needed and directed by the supervisor.


Supervisory Duties and Responsibilities

  • This job may have supervisory responsibilities specifically for individuals with an LPC/S. This need is dependent upon the needs of the department. This will be determined by the department supervisor.

Qualifications

  • Master’s Degree in psychology, clinical mental health, or counselor education
  • Current SC license as an LPC-A
  • A minimum of 2 years clinical experience providing direct services to adults, children, adolescents and families and counseling experience
  • Current knowledge of evidence-based models for mental health and substance abuse treatment
  • Ability to deliver validated evidence-based therapy services to patients
  • Able to develop treatment plans and document patient contact and progress
  • Possesses a valid South Carolina driver’s license and clean driving record
