The Role
We are seeking a compassionate and organized Case Manager to provide comprehensive support to clients, connecting them with appropriate services and resources. In this role, you will assist clients in meeting their social, educational, and housing needs, coordinate appointments, and ensure follow-up on care plans, all while fostering a positive and supportive environment.

What You’ll Do

Identify client needs and make referrals to appropriate agencies.

Schedule and follow up on client appointments.

Provide or coordinate transportation and escorts as needed.

Assist clients with housing options and continuing education opportunities (e.g., G.E.D.).

Support aftercare services and provide assistance with intakes, assessments, groups, and individual sessions as assigned.

Issue passes with clinical supervisor approval.

Maintain accurate documentation and client records.

Perform other clinical and administrative duties as assigned.
Qualifications

Bachelor’s Degree in Human Services or related field required.

CADC or CADC McBAP Development Plan required.

Strong organizational skills with the ability to manage multiple tasks simultaneously.

Ability to work under pressure and adapt to changing priorities.

Proficiency in computer applications (Zoom, Teams, Google Meet, Microsoft Office; NextStep training provided).

Valid driver’s license required.

Knowledge of gambling counseling recommended for Master’s-level clinicians/case managers.

Ability to pass MDOC LEIN background check.
-
Willingness to perform a variety of clinical and office tasks as needed.
