Responsibilities:
- Conduct thorough patient assessments and develop individualized care plans.
- Perform physical examinations and diagnostic tests to evaluate patient health.
- Prescribe medications and treatment plans in accordance with best practices.

- Collaborate with physicians and healthcare teams to ensure optimal patient outcomes.
- Maintain accurate and up-to-date patient records and documentation.

- Stay current with medical advancements and best practices in family medicine.
Requirements:
- Master's degree in Nursing and valid Family Nurse Practitioner certification.
- Current New York State Nurse Practitioner license.
- Minimum of 3 years of clinical experience in a family practice setting.
- Strong communication and interpersonal skills for patient engagement.
- Ability to work collaboratively in a multidisciplinary team environment.
- Knowledge of electronic health record (EHR) systems. eClinicalWorks a plus
