Job DescriptionJob Opportunity: Physical Therapist
Our organization is in search of a dedicated and empathetic Physical Therapist to become a vital member of our Healthcare & Medical Services team. In this position, you will deliver exceptional, evidence-based physical therapy services to patients throughout various care settings, including skilled nursing, rehabilitation, home health, and hospice support.
Key Responsibilities
- Conduct comprehensive evaluations of patients with a range of medical and functional requirements.
- Develop personalized treatment plans aimed at restoring function, alleviating pain, enhancing mobility, and improving overall quality of life.
- Perform thorough assessments and collaborate with patients, families, and the interdisciplinary care team to establish realistic and measurable goals.
- Regularly reassess patient progress and modify treatment plans as necessary.
- Administer therapeutic exercises, gait and balance training, functional mobility training, and neuromuscular re-education.
- Provide education to patients and caregivers tailored to individual abilities and objectives.
- Accurately document all evaluations, treatments, and patient responses in our electronic medical record system, ensuring compliance with regulatory and professional standards.
- Collaborate closely with physicians, nurses, occupational and speech therapists, social workers, and other team members to coordinate patient-centered care plans and facilitate safe discharges or transitions.
- Contribute to fall-prevention strategies, recommend equipment and assistive devices, and assist with home safety planning as needed.
- Educate patients, families, and caregivers on safe mobility techniques, proper use of assistive devices, energy conservation, and home exercise programs to promote long-term independence.
