We are seeking qualified candidates for the position of Rehabilitation Counselor. The position is responsible for providing direct support, counseling, and supervision to all assigned residents, ensuring a safe, healthy, secure, and pleasant environment that promotes growth and development as a member of the CRR program. Facilitate client growth and development toward independence in self-care, daily living skills, socialization, and community living. Please participate in the development of the individual service plans and related documentation, and document resident progress as they move through the service delivery system.
Other responsibilities include the following:
•Provide guidance and support to all participants, teaching and reinforcing skill development in areas such as nutrition, basic self-care, routine medical care, and follow-up, as outlined in the individual support plans developed through joint planning sessions.
• Review and modify individual service plans to accommodate growth and development.
• Ensure the overall safety of assigned participants by maintaining a hazard-free environment and avoiding potentially dangerous situations.
• Track all participant progress, as identified in their individual service plans. Provide supplemental reports on individual growth and development, as required. Record their progress as they move through the service delivery system.
• Serve as an advocate and encourage participants to assume a proactive role in their rehabilitative process. Attend and participate in case conferences to discuss participants' progress, needs, and concerns.
Qualifications:
• A.A. degree in the Social Sciences Healthcare or a related field and at least 1 year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
• Demonstrated crisis intervention/prevention and de-escalation skills required.
• Proficient communication skills, both oral and written.
• Ability to work independently and as part of a team.
• Some knowledge of the mental health system and supports.
• Valid driver’s license with an acceptable driving record required.
Scope:
• Frequent talking and listening.
• Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents.
• Possess manual dexterity and fine motor skills.
• May require crisis intervention in emergencies.
• May be required to report to work during emergencies, including inclement weather.
