This full-time Nurse Practitioner role is ideal for an NP who enjoys providing comprehensive primary care to adult and older adult patients.
You will have the stability of a clinic setting, the support of a collaborative team, and the opportunity to manage a mix of chronic and acute conditions with a strong focus on patient education and prevention.

What You Will Do
- Provide primary medical management in collaboration with physician partners
- Conduct physical exams, assessments, and diagnostic testing
- Create and implement individualized care plans
- Manage chronic conditions and episodic illness
- Educate patients on disease prevention and self-management
- Initiate referrals and coordinate care with specialists
- Support students and participate in team and departmental meetings
What You Will Bring
- Active Nurse Practitioner certification
- Current NP license in Massachusetts
- Experience working with adult and geriatric patients
- Ability to perform minor procedures and interpret diagnostics
- Strong communication, patient education, and documentation skills
- Comfort working independently while collaborating with the care team
- Multilingual ability in Spanish, Portuguese, or Cape Verdean is a plus
