The Primary Care Physician (PCP) in our organization is expected to:
- Take Responsibility for Outcomes: The PCP is accountable for delivering strong clinical care and cost-effective outcomes for a panel of up to 450 patients. They recognize the importance of fostering trusting relationships to positively influence patient outcomes.
- Act as a Health Coach: Rather than merely consulting on illness, the PCP helps patients establish both short- and long-term health goals, collaborates with them to achieve these goals, and regularly follows up to ensure progress.
- Simplify for Action: The PCP prioritizes tasks effectively to facilitate actionable behavior changes for patients and guides their care teams towards achieving performance objectives.
As an organization focused on outcomes and value, we regularly assess various metrics for each PCP's patient panel, including patient admissions per thousand, appointment utilization (18-21 slots daily), CGCAHPS (patient experience), clinical gap closures, and medical cost measures. PCPs will have specific targets for these metrics and will collaborate with center and market leadership, as well as their care teams, to meet these goals. A positive culture is vital in our medical centers, and as leaders, PCPs are expected to foster an environment of love, accountability, and passion alongside center leadership.
The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with relevant managers and medical directors to ensure adherence to guidelines and participate in risk and quality management programs, clinical meetings, and other necessary gatherings that promote patient health and organizational objectives. Compliance with departmental goals, performance standards, regulatory requirements, and quality patient care policies is essential.
KEY JOB DUTIES AND RESPONSIBILITIES:
- The PCP independently manages care for patients with both acute and chronic conditions, particularly in older adults.
- They take full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining involved in the patient's comprehensive care.
- Engagement with hospitalists is expected whenever their patients are hospitalized, regardless of the hospitalist's affiliation.
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex health needs.
- Leading a care team that includes a medical assistant, care facilitator, and care coordinator, the PCP ensures effective care for patients who can visit the office.
- For patients unable to attend the office—whether in hospitals, skilled nursing facilities, long-term care, or homebound—the PCP collaborates with transitional care teams and other resources as needed.
- The PCP plays an active role in managing their center and may cover for other providers as necessary. They are also expected to assist in patient recruitment for the center and the organization.
- Supervising and collaborating with Advanced Practice Practitioners (APPs), the PCP remains accountable for their actions while employed with the organization.
- Additional duties may be assigned at the discretion of management.

- Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, as well as other relevant software.
- Willingness to travel locally as needed, and occasionally regionally or nationally for training or recruitment purposes.
- Fluency in English, both verbal and written; some positions may require proficiency in additional languages, which will be specified during the application process.
- This role necessitates the exercise of independent judgment.
- Flexibility to work evenings, weekends, and/or holidays as required.
EDUCATION AND EXPERIENCE REQUIREMENTS:

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required.

A current, active State Medical License for the state(s) of practice is mandatory.

Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is preferred; board eligibility is required.

Once board certified, the PCP must maintain certification through necessary MOC, CME, and/or retaking board exams as required.

A current DEA number for controlled substances (schedules II-V) is required.

Basic Life Support (BLS) certification from the American Heart Association or American Red Cross is required within the first 90 days of employment.
SALARY RANGE:
$214,700 - $306,714
