Physical Therapist Responsibilities:
<ul>
<li>Assessing and interpreting patient evaluations and test results.</li>
<li>Developing treatment plans using a variety of treatment techniques.</li>
<li>Creating personalized rehabilitation programs for patients.</li>
<li>Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.</li>
<li>Provide clinical support and education to patients, family members, and caregivers.</li>
<li>Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.</li>
<li>Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.</li>

Requirements:
Physical Therapist Qualifications:
<ul>
<li>Active/Valid Illinois license in Physical Therapy</li>
<li>SNF/Long term care experience preferred.</li>
<li>Current knowledge of treatment practices</li>
<li>Knowledge of Net Health Electronic Documentation System preferred.</li>
