PCHC’s Hope House Health & Living Center is seeking a Transitional Housing Navigator to join our team. In this meaningful, hands-on role, you’ll support individuals living in transitional housing by offering compassionate guidance, helping maintain safe and welcoming living spaces, and connecting tenants to vital services and resources. You’ll play a key role in fostering a respectful, supportive environment where residents are empowered to build stability, confidence, and greater independence. Schedule: Part-Time (Anticipated 30 Hours/Week) | Saturday, Sunday, and Monday | Schedule May Vary

Highlights of the position:

  • Engages directly with individuals seeking residential housing services, fostering a supportive community atmosphere and treating all humans with kindness, dignity, and respect.
  • Is a good steward of confidentiality and discretion. Collects, completes, processes, and files all applicable intake documents for tenants, maintaining a thorough understanding of all necessary documentation.
  • Assists tenants with reasonable requests and connects them to services and resources as appropriate.
  • Provides consistent oversight of tenants, addressing behavioral and/or safety concerns in alignment with policies. Responds to and manages crisis situations with established protocols, prioritizing tenant safety and well-being.
  • Performs basic housekeeping and additional support functions, including but not limited to:
    • Preparing rooms for new tenants
    • Ensuring shared spaces are clean, sanitary, and well-maintained.
    • Distributing meals to tenants, ensuring timely service and equitable access.
  • Collaborates to ensure services are mission-focused. Continuously looks to seek improvement and implement best practices.

Education and Experience:

  • High school graduate or equivalent preferred
  • Experience and/or education in Mental Health or Chemical Dependency is preferred.
  • Will have up-to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
