Core Direct Care Duties

• Assist clients with bathing, dressing, grooming, and toileting

• Support with feeding and hydration, ensuring dietary needs are followed

• Help clients with mobility, including walking, transfers, and repositioning to prevent skin breakdown

• Take and record vital signs such as temperature, pulse, respiration, and blood pressure

• This job requires you to be a Certified HHA or have your CNA license.

Daily Living & Comfort

• Provide companionship and engage clients in conversation and social interaction
• Assist with meal preparation and encourage healthy eating habits
• Help maintain a clean, safe environment through light housekeeping
• Support with laundry and linen changes
• Assist with apparel selection and personal appearance
• This job requires you to be a Certified HHA or have your CNA license.

Observation & Reporting

• Monitor and report changes in physical condition, behavior, or cognitive status to supervising nurses
• Document daily care activities and client progress
• Identify and communicate any safety concerns or unusual symptoms
• This job requires you to be a Certified HHA or have your CNA license.

Mobility & Safety

• Ensure safe transfers using gait belts or other approved equipment
• Assist with ambulation and fall prevention strategies
• Maintain awareness of client safety at all times
• This job requires you to be a Certified HHA or have your CNA license.

Client Engagement

• Participate in clients’ preferred activities such as games, gardening, crafts, or outings
• Accompany clients on errands or appointments, as permitted by the employer
• Other duties as assigned by Manager as this is not an exhaustive list.
  
  *Compensation for this position is $15.50/Hr*
