We are currently seeking an experienced MDS Coordinator to join our team at a skilled nursing facility. This role is critical in ensuring accurate assessments, compliance, and optimal reimbursement.
Responsibilities:

Complete and oversee MDS assessments and care plans
</li>

Ensure accuracy and timeliness of all submissions
</li>
<li>
Maintain compliance with Medicare/Medicaid regulations
</li>
<li>
Work closely with interdisciplinary team
</li>
<li>
Monitor case mix and reimbursement optimization
</li>
<li>
Assist with audits and survey readiness
</li>
</ul>
Qualifications:
<ul>
<li>
Active RN or LPN license required
</li>
<li>
Prior MDS experience in long-term care required
</li>
<li>
Strong knowledge of RAI process and PDPM
</li>
<li>
Detail-oriented with strong organizational skills
</li>
<li>
Ability to work independently and meet deadlines
</li>
