Job DescriptionJob Summary
The RN Case Manager at Josephs House drives comprehensive care delivery within a multidisciplinary framework.

Collaborate: Partner with social workers, clinical coordinators, and staff to address the diverse needs of the community.
Coordinate: Manage the full lifecycle of member services, including intake, assessment, and complex discharge planning.
Advocate: Provide integrated medical and non-medical case management.
Connect: Build strong relationships with leadership, residents, and external community partners to enhance service quality.

Job Responsibilities
The selected applicant will serve as the lead registered nurse managing resident care. Key responsibilities include:

Admissions & Triage: Lead the triage and admission process for new residents, ensuring clinical alignment with the communitys care capabilities.
Clinical Oversight: Manage comprehensive resident care, including complex symptom management, medication administration, and the supervision of daily personal care.

Protocol Development: Design, implement, and update resident care protocols to maintain high standards of clinical excellence and safety.

Continuum of Care: Provide specialized medical case management for residents at all stages, from medical stabilization and community reintegration to compassionate end-of-life care.

Interdisciplinary Coordination: Act as a primary liaison and resident advocate with hospitals, hospices, and outpatient clinics to ensure seamless transitions and continuity of care.

Mentorship & Supervision: Train and supervise resident care staff and volunteers to ensure the delivery of high-quality, patient-centered support.

Patient Advocacy: Accompany residents to medical appointments as needed to facilitate clear communication and advocate for their health needs within the broader healthcare system.

Operational Readiness: Participate in a shared weekend and overnight on-call rotation to ensure 24/7 clinical support for the community.

Qualifications
Registered nurse licensed in the District of Columbia
At least five years of nursing experience
Experience working with vulnerable populations, and a desire to bring ones whole self to the service of our community
An ability to remain peaceful and productive in times of stress, and to collaborate effectively in a multicultural community

Pay:
This position starts at $43/ Hour DOE.
