Location: Pine Creek Rehab - Salt Lake City, UT

Pine Creek Rehab is seeking a passionate and experienced Director of Nursing (DON) to lead our clinical team. We are a skilled nursing facility (SNF) committed to delivering exceptional resident-centered care and supporting each patient in achieving their highest level of independence before transitioning to the next phase of care.

✨ What You'll Do

  • Oversee the development and execution of all nursing programs and clinical activities

  • Create, maintain, and routinely update written nursing policies, procedures, manuals, objectives, and care philosophies

  • Collaborate with other departments to ensure seamless coordination of resident services

  • Identify, implement, and monitor corrective action plans when deficiencies are identified

  • Accurately complete required medical documentation, reports, studies, evaluations, and charts

  • Develop, implement, and lead a comprehensive Quality Assurance (QA) program

  • Support planning, implementation, and evaluation of discharge processes and resident transition plans

  • Establish and manage an effective nursing department organizational structure

  • Participate in federal, state, and facility compliance surveys and inspections

  • Prepare and present departmental reports and recommendations to the Administrator

???? Requirements

  • Associate degree in nursing required; Bachelor's degree preferred

  • Active Registered Nurse (RN) license in good standing

  • Minimum 1 year of successful nursing leadership experience in long-term care or geriatric healthcare

  • Strong written communication skills with clear and legible documentation

  • Must speak and understand English

  • Must demonstrate genuine compassion and commitment to working with the geriatric population
