JOB SUMMARY:

The Community Relations Coordinator supports the clinic(s) by the implementation of the Community Relations Department’s objective to carry out the mission of CHSI through developing community partnership and relationships, organizing clinic resource events, and educating the community about healthcare system navigation and social determinants of health.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Upholds and supports the mission, philosophy, objectives, policies and ethics of CHSI.

2. Will smile, speak in a courteous and friendly manner with patients, staff members and clients.

3. Protects organization’s value by keeping information confidential.

4. Provide timely feedback to direct manager and work in collaboration with the Clinic Administrator to implement the Community Relations Department’s specific strategic plan to increase awareness and education on healthcare system navigation.

5. Meets regularly with the Clinic Administrator and designated staff to help identify and/or communicate community issues, feedback, service needs, events, and the Community Relations Department’s strategic plan.

6. Comply with expectations of grant requirements for the purpose of statistical tracking to generate monthly reports.

7. Serve as a liaison between the Community Relations Department, health center staff, and the community while continuing contact with social networks and community partners.

8. Assist Community Relations Supervisor in establishing venues to generate community partnerships opportunities through existing and new accounts, cold calling and educational presentations.

9. Ability to find resources to assist patients with the needs outside the scope of our services at CHSI.

10. Acts as a liaison in performing value-added activities, customer service and service recovery to ensure patient satisfaction

11. Continuously updates the designated Community Relations Coordinator that administers CHSI social media educational campaigns.

12. Responsible for participating in quality improvement projects/activities and accountable for quality and PCMH.

13. Perform all other duties as directed either formally or informally, verbally or in writing.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Bilingual (English/Spanish) strongly preferred
  • Strong analytical and critical thinking skills; ability to pay close attention to detail.
  • Strong computer skills, with proficiency in Microsoft Office
  • Well-developed verbal and written communication skills and the ability to work cooperatively with divergent and diverse groups
  • Must possess good customer service skills including verbal and written communication.
  • A high standard of professionalism is expected in speech, manner, attitude and appearance at all times.
  • Possess strong interpersonal skills and ability to work well with others.
  • High standard of professional ethics and conduct is expected.
  • Ability to deal effectively with changing situations and stressful environment.

· Ability to perform tasks related to physical activity to complete the responsibilities of the position.

· Be able to lift up to 50 lbs. with the help of another person.

EXPERIENCE AND EDUCATION:

· Three years or more of related experience required. Bachelor Degree or five years of experience preferred.

· Current CA Driver’s License and proof of automobile insurance

· Must obtain a Certified Enrollment Counselor (CEC) certification within 60 days of employment

· Must be able to travel locally 60% of the time
