Role Overview: In this pivotal role, you'll provide crucial support and expert medical care to patients in their homes or designated facilities. You'll leverage your compassion and expertise to guide families through their experiences, ensuring comprehensive care by coordinating with an interdisciplinary team.

Key Responsibilities:

  • Patient Care: Perform thorough assessments to address the physical, emotional, and holistic needs of each patient.
  • Care Coordination: Develop personalized care plans that respect the preferences of patients and their families.
  • Communication: Act as the essential communicator among physicians, patients, and caregivers.
  • Education: Educate and support families in essential self-care techniques.
  • Leadership: Maintain a safe and supportive environment, setting a high standard of care.
  • Team Collaboration: Participate actively in interdisciplinary team meetings to enhance patient care.
  • On-Call Support: Provide dependable support as needed, guided by the Clinical Manager.

Who We Are Looking For:

  • Experience: Minimum of 2 years as a Registered Nurse, with a preference for 1 year in Hospice Care.
  • Licensure: Must be a Registered Nurse in Pennsylvania with valid CPR certification.
  • Skills: Exceptional organizational and communication skills are essential.
  • Transportation: Must have a vehicle, a valid driver's license, and insurance.
