Job DescriptionDescription:

The Home Care Physical Therapist (PT) is responsible for providing skilled physical therapy services to patients in their homes. The PT evaluates patients, develops individualized treatment plans, and implements therapy programs to improve mobility, strength, and overall physical function. The goal is to promote independence and enhance the quality of life while ensuring patient safety.

Requirements:

Key Responsibilities:

Patient Care & Treatment:

• Conduct initial assessments to evaluate the patient’s physical abilities, mobility, and functional limitations.

• Develop and implement individualized treatment plans based on patient goals and physician recommendations.

• Provide therapeutic exercises, balance training, and pain management techniques.

• Educate patients and caregivers on home exercises, injury prevention, and proper body mechanics.

• Monitor and adjust treatment plans as needed to ensure progress toward goals.

• Document all treatments, progress notes, and changes in the patient’s condition in a timely manner.

• Ensure compliance with all home health regulations and company policies.

Collaboration & Communication:

• Coordinate care with physicians, nurses, occupational therapists, and other healthcare professionals.

• Provide guidance and education to caregivers and family members regarding patient care.

• Communicate effectively with the healthcare team to ensure optimal patient outcomes.

Compliance & Safety:

• Follow federal, state, and local regulations for home health physical therapy services.

• Ensure all interventions prioritize patient safety and adhere to best practices.

• Maintain confidentiality and uphold HIPAA regulations.

Qualifications:

Education: Doctorate (DPT) or Master’s degree in Physical Therapy from an accredited program.

Licensure: Active and unrestricted Physical Therapist license in CA.

Experience: Prior experience in home health, geriatric care, or outpatient therapy preferred.

Certifications: CPR/BLS certification required.

Skills: Strong communication, time management, and problem-solving skills. Ability to work independently and adapt to different home environments.

Physical Requirements:

• Ability to lift, push, or pull up to 50 lbs.

• Frequent standing, walking, bending, and kneeling.

• Ability to drive to multiple patient homes within the designated service area.
