Job Duties:
- Patient Care:
- Work with geriatric patients
- Complete Medicare annual health assessments at clinics and home visits
- Location: San Diego
- Patient Load:
- 7-8 patients/day in clinic
- 3-5 patients/day on home visit days
- This position will be mostly home visits with some appointments in clinic.
- Clinical Responsibilities:
- Perform H&Ps
- Use preventative health screening tools (PAD and Neuropathy, diabetic retinal cameras, A1c and kidney testing for diabetics)
- Navigate and chart via EMR systems
- This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role.

- Cell phone reimbursement
Compensation:
- 150K-170K
- Additional travel opportunities available with premium pay
Requirements:
- Current NP license for the state of California
- Ability to use a computer efficiently
- EMR systems and electronic documentation
- Bilingual (Spanish required)
