Role Summary
As Director of Nursing and Care Management, you’ll be the cornerstone of our Detroit launch. You’ll start as a player-coach: delivering direct care in clients’ home and senior living communities, while laying the groundwork for operations, systems, and team development.
WellSpring Care Managers provide end-to-end support for older adults and their families — starting with a comprehensive assessment across clinical, functional, cognitive, and psychosocial domains. We then develop and implement a personalized care plan that may include skilled nursing, medication management, care coordination, health system navigation, support through transitions, and chronic condition management.
Over time, you’ll evolve into a full leadership role — overseeing clinical operations, mentoring staff, and ensuring gold-standard care for every client. You won’t be building alone. You’ll partner closely with the WellSpring Home Office, with strategic and operational support every step of the way.
The Details: Key Responsibilities
Client Care & Clinical Leadership
- Serve as the lead Nurse Care Manager during the early growth phase
- Conduct client intakes, comprehensive assessments, and ongoing care planning
- Transition into a leadership role focused on clinical oversight and assessments as staff is hired
- Ensure high standards of care, communication, and documentation are maintained
Team Building & Staff Management
- Recruit, hire, onboard, and support Care Managers
- Provide coaching, supervision, and performance management
- Foster a collaborative, accountable, mission-driven team culture
Operational Implementation
- Launch and manage clinical and administrative systems using the WellSpring Playbook
- Oversee scheduling, workflows, and reporting until support roles are added
- Support financial performance goals, including healthy margin targets and service growth
Business Development
- Support local business development efforts to help launch and grow the new market. Initial outreach will focus on partnership-based referrals (home care, senior living, hospitals, physicians, etc), particularly through WellSpring’s established partners
Who You Are:
- A licensed Registered Nurse (RN) with at least 5 years of clinical experience in home care, hospice, geriatric case management, or other community-based senior care settings
- A strong people leader who enjoys hiring, mentoring, and team building
- Highly organized and process-oriented, with a knack for systems thinking
- Energized by growth and autonomy — and comfortable building in a start-up environment
- Experienced or interested in business development and stakeholder engagement (a plus)
